Before you fill out an appointment request, please make sure you fill out the client/patient form first, then request your next appointment conveniently on this page. Simply choose a date and time that work best for you, and we'll be in contact shortly. While we try to accommodate everyone's first choice time slot, sometimes it isn't possible.
